To assemble application, you must have a trusted profile and on the www.gov.plpage, log in my GOV account (upper right corner of the page). Later select e-service Get a copy of the civil status certificate (birth, marriage, death) and that the document we want to receive my GOV.

System will generate the document directly from the State Register System (SRP) and then send it to your inbox.

Edition document by the vending machine will be possible under several conditions.

• a copy of the act, you must be concerned with you, your children, your spouse or your Parents
• system - in application, it must confirm that it is possible to automatic release of a write-off; this applies to acts drawn up since March 2015 year or transferred from paper books to the SRP,
• Payment for the write-off must be made online: PLN 22 – for a summary of the state (also for a multilingual copy in paper form), PLN 33 – for a full copy of the civil status certificate. If you're doing the trick by proxy – you will pay an additional PLN 17 for the power of attorney.

If the system will not release your copy automatically and you will receive it anyway.

You can find the finished copy in your inbox your My GOV account www.GOV.pl. corner of the page) and you'll get to it when you sign in with a trusted profile.

The copy you get will have the power to only in electronic form. Do not print it and pass it only in this Form. The printout has no legal force. The automatic copy will contain a signature authenticity of the copy.
